The decision means that any female will be able to buy Plan B One-Step in any drugstore without a prescription.</p>

<p itemprop="articleBody">A U.S. District Court judge had already ordered the Food and Drug Administration to certify Plan B One-Step for nonprescription use. The issue would have been headed to the Supreme Court if the Obama administration appealed, which would further flame political tensions over the controversial issue. Up until now, the pill was only available without a doctor&#8217;s note to women 17 and older.</p>
<p itemprop="articleBody">“This is a huge breakthrough for access to birth control and a historic moment for women’s health and equity. The FDA’s decision will make emergency contraception available on store shelves, just like condoms, and women of all ages will be able to get it quickly in order to prevent unintended pregnancy,&#8221; <a title="Planned Parenthood Hails Obama Decision to Drop Opposition to Morning-After Pill" href="http://www.plannedparenthood.org/about-us/newsroom.htm" target="_blank">said Parenthood Federation of America President Cecile Richards</a>. &#8220;Emergency contraception is a safe and effective form of birth control that can prevent pregnancy if taken within five days of unprotected sex.&#8221;</p>
<p itemprop="articleBody">&#8220;This decision by the Administration affirms what feminists have been fighting for all along – the Morning-After Pill should be available to females of all ages, on the shelf at any convenience store, just like aspirin or condoms,&#8221; said<br />

<p>In 1992, Dr. James Trussell and the late Dr. Felicia Stewart, authors of <em>Contraceptive Technology</em>, projected that widespread use of ECPs [emergency contraception pills] could prevent half of all unintended pregnancies and abortions in the U.S. each year. Unfortunately, that has not been the reality. In fact, in over 14 studies where the impact of ECPs has been examined, none have shown any benefit.</p>
<p>The issue isn’t that ECPs don’t work; studies clearly show that both the levonorgestrel products (Plan B and Next Choice) taken within 72 hours of unprotected sex alters the coarse of ovulation, thereby preventing conception. The levonorgestrel products are available over the counter to women 17 and older. The newest EC product, ullipristal acetate (Ella) is available only by prescription and can prevent ovulation up to five days.  For the greatest efficacy in delaying ovulation, the EC products should be taken as soon as possible after unprotected sex.</p>
<p><strong>That something else may be access to the products and being educated that these products exist and when to use them</strong>.</p>

<p>An “emergency” is not a good time to do the research about what to do or which pharmacies stock the medication or are open on a weekend. <strong>Advanced prescriptions, as the AAP has suggested, makes sense. It serves as a reminder that there is a second chance to prevent pregnancy.</strong> Advance prescriptions can also mean an adolescent (or women of any age) can get EC to have on hand “just in case.”</p>

More stories about random shootings make momentary headlines. And although there is still some news about the quaint Connecticut town soon, I am afraid, the <a title="For more stories on Newtown shooting" href="http://www.genconnect.com/tag/newtown-shooting/" target="_blank">Newtown tragedy</a> will fade as things get back to “normal.&#8221;</p>

<p>Since the shootings we have heard from the commentators, police, mental health specialists, the clergy, and yes, the NRA [National Rifle Association] for solutions. I have heard countless individuals punting the problem to someone or something else. On Facebook and with every teddy bear and candle left for the victims, we prayed. It makes us feel like we are doing something. However, punting the problem to God or anything else, does little to address the real issue.</p>
<p>Some suggest we provide better mental health services to identify would be killers. Again, passing the problem over. Let the government and health care system figure this out.<strong> True, mental health services could be significantly improved. But it isn’t the solution.</strong></p>
<p>Others blame violent video games, TV, and movies. And while I agree that these pastimes might contribute to the objectification of human beings, not everyone who has ever watched a violent movie or played a violent video game ends up a mass murderer. Hardly. Plus, video games, TV, and movies are not something that most of us are in a position to change.</p>

<p>The NRA’s solution to put guns in the hands of good guys and security in the schools is the most transparent &#8220;it’s not me&#8221; solution out there. It isn’t a gun problem that creates mass murders. However, why anyone needs a semi-automatic with clips that can kill many in seconds elude me. When the Second Amendment was written, it was one gun and one bullet at a time. The NRA needs to give this one up. But we still won’t stop the killing.</p>
<p><strong>The bottom-line problem: It’s us. We live in a society in which humanity is someone else’s problem. &#8220;</strong>I am fine,&#8221; we say to ourselves.  We don’t recognize the violence we are capable of committing every day. Many are not willing to look inside to see how violence can manifest in the things we accept as &#8220;normal.&#8221;</p>
<p>Violence isn’t just physical. Any action in which a person is seen as only an object and a lesser human being is violent by itself. Look at some random acts of violence that don’t require an AK-47. How about: Gossip? Hurtful back-talking? Blaming? Bullying? Shouting in anger to get a point across? Bemoaning another’s accomplishments, hopeful they will be &#8220;taken down a few pegs&#8221; — as I was once told. How hurtful those things are.  How violent. Yet we accept these things as &#8220;normal” &#8211; or at least not violent.</p>

<p>We pass on the gossip and feel superior doing so. Soon gossip becomes truth when it is repeated enough. <strong>Our culture has taught that we can boost ourselves by the demise of another. Unless we are willing to change that behavior in ourselves we are lost</strong>. None of us are perfect. Many don’t even recognize their behavior as violent. Perhaps if we just pledged to ourselves to catch those random acts of violence toward another person we could condition ourselves to be more compassionate the next time. If we don’t, we will just continue to accept that hurting others, in whatever form, is A-OK. We can’t afford to foster the next mass killer by allowing everyday violence.</p>
<p><strong><a href="http://www.genconnect.com/wp-content/uploads/2013/01/hugyourlovedone1.jpg"><img class="alignleft size-medium wp-image-41687" title="hugyourlovedone" src="http://www.genconnect.com/wp-content/uploads/2013/01/hugyourlovedone1-300x224.jpg" alt="" width="300" height="224" /></a>So instead of pushing the problem off to the government, the police, the mental health specialists, the gun dealers, and God, let’s pledge to ourselves to take on building a world with more compassion and love for humanity. It is the one thing we as individuals can do. </strong></p>
<p>It&#8217;s the best New Year’s resolution I can think of.</p>

Think again.</p>
<p>The <a title="Emergency Contraception Should Be Available to Teens, AAP Says" href="http://www.aap.org/en-us/about-the-aap/aap-press-room/Pages/AAP-Recommends-Emergency-Contraception-Be-Available-to-Teens.aspx" target="_blank">American Academy of Pediatrics (AAP) this week recommended </a>that its members provide emergency contraception or &#8220;the morning after pill&#8221; &#8211; to “teenagers in immediate need” and also write prescriptions in advance for emergency contraception “for teenagers to have on hand in case of future need.”</p>

<p>over the counter for all adolescents based on safety and findings that adolescents could follow the directions. Further, the FDA panel recognized that access to the over-the-counter product did not increase risky behavior among teens. The FDA’s approval was revoked by Department of Health and Human Services Secretary Kathleen Sebelius in an unprecedented move by the Obama administration.</p>
<p><em><strong>Emergency contraception does not work if it is not used.</strong> <strong>It works less effectively if there is a time delay.</strong></em></p>

<p>Not every pharmacy or pharmacist provides emergency contraception. Advanced prescriptions make common sense. The reality is that accidents and unprotected sex happens all the time. Let’s hope that pediatricians and other health providers for reproductive aged women of all ages take note and help give their patients information about this second chance to prevent an unwanted pregnancy.</p>
<p>Along with health care providers, moms and daughters (and sons) or a close family member can share this information, as well. I talked to my niece about emergency contraception early in adolescence. It was the perfect opportunity for me to tell her why I hoped she would never have to use it. <p>As for the AAP, the organization <a title="Emergency Contraception Policy Statement" href="http://pediatrics.aappublications.org/content/early/2012/11/21/peds.2012-2962" target="_blank">says in its new policy statement published in the journal, <em>Pediatrics</em></a>: &#8221;Despite significant declines over the past 2 decades, the United States continues to have teen birth rates that are significantly higher than other <a href="http://www.genconnect.com/wp-content/uploads/2012/11/AmericanAdademyPediatrics.jpg"><img class="alignleft  wp-image-40012" title="AmericanAdademyPediatrics" src="http://www.genconnect.com/wp-content/uploads/2012/11/AmericanAdademyPediatrics-300x300.jpg" alt="" width="210" height="210" /></a>industrialized nations. Use of emergency contraception can reduce the risk of pregnancy if used up to 120 hours after unprotected intercourse or contraceptive failure and is most effective if used in the first 24 hours.&#8221;</p>

<p>Currently, emergency contraception like Teva Pharmaceutical&#8217;s Plan B One Step, is available to adolescents 17 and younger only with a doctor&#8217;s prescription in most states, but females 17 years or older and males 18 years or older can obtain emergency contraception without a prescription in every state the Academy says &#8220;<strong>adolescents are more likely to use emergency contraception if it has been prescribed in advance of need.&#8221; </strong></p>
<p>The Academy&#8217;s recent recommendation seeks to skirt a federal ban on over-the-counter sales of emergency contraception to girls under 17 and to educate physicians on the use, safety and efficacy of these pills. The U.S. Food and Drug Administration in December 2011 rejected a move to allow emergency contraception to be sold over the counter to girls younger than 17.</p>
<p>The American Congress of Obstetricians and Gynecologists <a href="http://www.acog.org/Resources_And_Publications/Committee_Opinions/Committee_on_Gynecologic_Practice/Over-the-Counter_Access_to_Oral_Contraceptives" target="_blank">also recommended</a> that oral contraceptives be available over the counter.</p>

<p><strong><em>Birth control for men: New research shows that the JQ1 molecule can make men temporarily infertile, which could help formation of effective male contraception; Susan Wysocki of iWoman&#8217;s Health weighs in </em></strong></p>
<p>Could a male birth control pill really be heading toward a pharmacy near you? Quite possibly, according to recent news.</p>
<p>While working on cancer inhibitor research, James Bradner, of Harvard University&#8217;s Dana-Farber Cancer Institute, says he realized that molecule, named JQ1, can also inhibit a protein in the male testes that is imperative to fertility. So far, this has only been tested in mice, but the findings could be revolutionary, and <a href="http://www.usnews.com/news/articles/2012/08/16/male-birth-control-pill-could-be-ready-for-testing-in-a-year" target="_blank">we could know just how effective it is in humans in about a year.</a></p>

<p><strong>So what would this mean for the way women and men approach contraception and birth control? </strong><strong>We reached out to <a href="http://www.genconnect.com/author/susan-wysocki/" target="_blank">Susan Wysocki</a>, WHNP, FAANP and President at iWoman&#8217;s Health to get her thoughts. Here&#8217;s what she said&#8230;</strong></p>

<p><strong>One of the most common questions I get when lecturing about contraception is &#8220;when will there be a method for men?”</strong></p>
<p>The discovery that JQ1 originally being tested for treatment of cancer but found to inhibit sperm production sounds the most promising yet. But it will be a long time before it is tested and even longer, assuming it works, to get it to market. Let’s hope there is something for men.</p>

<p><strong>The most common comment I hear is, &#8220;Yes, but would you trust a man to say he is protected?&#8221;</strong></p>
<p>Unless a man uses a condom, he is vulnerable to an unintended pregnancy and the responsibility that goes with it. Birth control methods for women are not perfect, particularly those that rely on correct and consistent use. So a man might ask the same question: &#8220;Can I be sure that she is using her birth control correctly?&#8221;</p>

<p><strong>Either way, a reversible male method of contraception would provide a shift in paradigm where men can think of contraception to protect themselves from unintended life-changing events</strong>. If there was something other than condoms or vasectomy, it could change the way that men look at the vulnerability they have relying on someone else for protection. That could bring about a major shift in how unintended pregnancy is viewed.</p>

<p><a href="http://www.genconnect.com/wp-content/uploads/2012/08/shutterstock_47584525.jpg"><img class="alignleft size-full wp-image-35041" title="egg and sperm" src="http://www.genconnect.com/wp-content/uploads/2012/08/shutterstock_47584525.jpg" alt="" width="300" height="225" /></a>&#8220;A pharmacologic approach to male contraception remains a longstanding challenge in medicine,&#8221; Bradner and Martin Matzuk, of the Baylor College of Medicine write in their study, published in the journal <a href="http://www.cell.com/abstract/S0092-8674(12)00929-4"><em>Cell, </em>revealing their findings<em>.</em></a><em> &#8220;</em>Treatment of mice with JQ1 reduced seminiferous tubule area, testis size, and spermatozoa number and motility without affecting hormone levels. Although JQ1-treated males mate normally, inhibitory effects of JQ1 evident at the spermatocyte and round spermatid stages cause a complete and reversible contraceptive effect. These data establish a new contraceptive that can cross the blood:testis boundary and inhibit bromodomain activity during spermatogenesis, providing a lead compound targeting the male germ cell for contraception.&#8221;</p>
<p>&#8220;These cells effectively forget to make mature sperm,&#8221; <a href="http://www.usnews.com/news/articles/2012/08/16/male-birth-control-pill-could-be-ready-for-testing-in-a-year" target="_blank">Bradner told U.S. News &amp; World Report.</a> &#8220;The result is a profound decrease in sperm count and impared motility, leading to a complete contraceptive effect. It&#8217;s really stunning. As early as next year, we may have a sense of how well this works in humans.&#8221;</p>

<p>Earlier this year, President Obama&#8217;s administration announced that most health insurance plans must cover free contraceptives for women as part of the rollout of the Patient Protection and Affordable Care Act, but church groups could opt out. After an outcry from religious groups and conservatives who said more religiously-affiliated organizations should also be given an allowance, Obama said other religious universities, hospitals and charities could also choose not to cover contraception in their health care plans.</p>

<p>Some states are searching for ways to block Obama&#8217;s new rule, or to otherwise allow more leeway for religious groups to halt coverage. Last week, for example, Arizona <a href="http://www.realclearpolitics.com/news/ap/politics/2012/May/12/brewer_signs_ariz__bill_on_contraception_coverage.html" target="_blank">Gov. Jan Brewer signed a bill</a> that gives religiously oriented employers the ability to halt contraception coverage for birth control purposes. These employers would still have to provide coverage for birth control used for other medical reasons; the bill also affects coverage for abortion-inducing drugs.</p>
<p><strong>Given that the issue of contraception so important to women&#8217;s health, we asked Susan Wysocki, President at iWoman&#8217;s Health and former CEO of the National Association of Nurse Practitioners in Women’s Health, to weigh in. Here&#8217;s what she said&#8230;</strong></p>

<p>While we won’t know until some time in June whether the Supreme Court will find the Accountable Care Act constitutional, unconstitutional, or simply parts of it unconstitutional, the debate about whether contraceptive care should be a covered service rages on.</p>
<p>Some argue that paying for contraception is paying for a lifestyle issue. Others have argued that women like Georgetown Law student <a href="http://www.genconnect.com/tag/sandra-fluke/" target="_blank">Sandra Fluke</a> could well afford to pay for her own birth control. Religious issues, central to much of the debate, argue that individuals should not have their taxpayer dollars pay for something that goes against their conscience. <strong>Even some women I have spoken with have said that they were able to prevent an unintended pregnancy by paying for their own contraceptives. Why shouldn’t other women?</strong></p>

<p><strong>According to the Guttmacher Institute, nearly half of the pregnancies in the United States are unintended. </strong>Some of those unintended pregnancies as mistimed, meaning that the woman would have preferred having a child at a later time in her life. About 20 percent of those pregnancies are unwanted. In 2006, the latest year that data are available, indicates that 43 percent of unintended pregnancy ended in abortion. <strong>The U.S. unintended pregnancy rate is higher compared to many other developed countries.</strong></p>
<p>There are few people who would argue that unintended pregnancy isn’t a problem. Although there are many disparate opinions about how the rate of unintended pregnancy can be reduced.  As someone who has been involved in helping to reduce unintended pregnancy for much of my career, I do know that access to contraceptive services helps. I also know that methods like intrauterine contraception (IUDs) and implants, are the most effective methods of reversible contraception, but they are the least covered by insurance now.  The methods require no user intervention&#8211;remembering to take that pill or use that condom&#8211;and thus, human error is taken out of the equation.  However,  many women cannot  afford the up front cost for these methods.  Considering the number of years of efficacy  these methods provide, over time they are the more cost effective than any other methods.  If all FDA approved methods of contraception are covered benefits, many more women would have the opportunity to choose these very effective methods.</p>

<p>What puzzles me the most about the arguments about taxpayer dollars and contraception is that the discussion often does not include the cost of unintended pregnancy to the taxpayer — roughly $11 billion. That’s $11 billion of unintended taxpayer dollars. Looking at the cost for individual subscribers of a health plan, the Washington Business Group on Health and the National Business Group on Health determined that when the medical costs of unintended pregnancies are taken into account, including the costs of prenatal care, pregnancy complications, and deliveries, the cost of providing contraceptives and contraceptive care is a zero sum cost.</p>
<p>Global Health Outcomes estimates that an employer paying for health benefits could save $97 per year, per employee due to the indirect costs such as loss of productivity due to an unintended pregnancy.</p>
<p><strong>Some may call me a liberal for supporting the coverage of contraceptive services. I am simply a taxpayer who wants my tax dollars at work doing something for women and saves us all billions per year.</strong></p>

<h2><strong>In case you missed the live chat, here are  highlights from the conversation:</h2>
<p></strong></p>
<ul>
<li>You <em><strong>can </strong></em>become pregnant even after you start experiencing signs of menopause! So don&#8217;t throw out those birth control pills, yet! Oral contraception (the pill), the contraceptive ring or patch are options for non-smoking women. &#8220;One advantage of these methods in your age group, is that they can also help with menopausal symptoms. Talk to your health care professional about these options,&#8221; Wysocki says.</li>
<li><strong>Be careful using oral contraceptives if you&#8217;re a smoker.</strong> &#8220;I am uncomfortable even with light smokers to recommend combined hormonal contraception&#8211;pill, patch ring,&#8221; Wysocki says. IUDs (intrauterine devices) such as <a href="http://www.mirena-us.com/" target="_blank">Mirena</a> or the <a href="http://www.paragard.com/" target="_blank">ParaGard</a> may be a better option.</li>
<li><strong>If you are on a depression medication such as Zoloft, you have many contraceptive options that shouldn&#8217;t cause you massive mood swings</strong>. There&#8217;s &#8220;no evidence to suggest they cause mood changes. If you find this happens, however, see your health care provider about switching methods &#8211; before you are at risk for pregnancy. Unintended pregnancy is a real mood dampener.&#8221;</li>
<li><strong>Got daughters in their early teens? Now&#8217;s a good a time as any to start talking to them about contraception.</strong> &#8220;You can start with what she wants to know. You might also consider getting her the HPV vaccine to protect her against cervical cancer, as well as the consequences of abnormal Pap tests.&#8221;</li>
<li><strong>You need more than female contraception to prevent the transmission of STDs</strong>. Condoms! Condoms! Condoms!</li>
<li><strong>For breastfeeding moms also on birth control, if you are exclusively breast feeding, you don&#8217;t need any extra protection for at least three months</strong>. Intrauterine contraception such as a progestin-only method (such as Depo-Provera or progestine-only pills) are also an option, as is the pill beginning one month after you deliver your baby.</li>
<li><strong>Most combined hormonal contraceptives (the pill and others) will help your skin</strong>. However, sometimes when women first start these methods they can get mild acne. The acne will typically go away and the skin improves for the better. Some pills have specifically been approved for treating mild to moderate acne. It may take time!</li>

<p>I think choosing a method of contraception should be like buying a new pair of shoes. We would never consider buying shoes after looking at only one pair. We wouldn&#8217;t consider going to the store that offered only one style or color. We don&#8217;t expect that our favorite pair of shoes when we are 20 will be our favorite pair when we are 30 or 40.</p>
<p>Yet, sometimes we choose and use methods of <a href="http://www.genconnect.com/relationships/the-pills-effect-on-our-sexual-health/" target="_blank">contraception</a> with less thought (and sometimes less information) than we do picking out a pair of shoes. Finding the right method of contraception, I dare say, is far more important than that pair of shoes. And, <a href="http://www.genconnect.com/lifestyle/how-many-pairs-of-shoes-do-you-own-poll/" target="_blank">I like shoes</a>.</p>
<p>Similar to shoes, there are contraceptive choices that are more or less appealing depending on your lifestyle and age. There are contraceptives for daily use, monthly use, or those that can be used for 5 years or more, but are still completely reversible. Also like shoes, most methods of contraception need a &#8220;breaking in&#8221; period. Starting a hormonal method, no matter what kind, usually takes a few months for minor adjustments. <a href="http://www.genconnect.com/wp-content/uploads/2011/05/BirthControl-ShoesPhoto.jpg"><img class="alignright size-medium wp-image-8763" title="BirthControl-ShoesPhoto" src="http://www.genconnect.com/wp-content/uploads/2011/05/BirthControl-ShoesPhoto-300x297.jpg" alt="" width="272" height="269" /></a>But they can become our most comfortable &#8220;shoes&#8221; after that. Most importantly, because there are many choices, there is never a time when a woman should go without effective contraception when she does not want to become pregnant. There are always other choices available.</p>
<p>Unlike shoes, where there are few myths and misunderstandings, there are many things that are believed about birth control methods that aren&#8217;t true. Do they cause weight gain? Do they cause difficulty getting pregnant when stopped? Do they cause cancer? The answer to these questions for methods of contraception is no, no, and no! With the possible exception of injectable contraception that may cause some weight gain for a woman who has already gained a considerable amount of weight regardless of whether they have ever used a contraceptive method.</p>
<p>Knowledge about choices, benefits, risks, and the right fit is as important for contraception as it is for getting those shoes that are too good to pass up.</p>

<p><em>&#8220;</em><strong>Great sex should not put someone at risk</strong> for an unintended pregnancy or a sexually transmitted disease, including HIV. Great sex is not so great if it creates worry and health problems later.&#8221;</p>
<p>&#8220;<strong>Great sex has many variations</strong>.  A recent report in the Journal of Sexual Medicine (JSM) by researchers at Indiana University reported 40 different combinations of sexual practices.  <em>Viva variation</em>.&#8221;</p>
<p>&#8220;<strong>Great sex does not hurt</strong>.  The same study in JSM reported that one third of women reported that sex hurt during their last sexual encounter.  Women should find a health care provider that will talk to them about sex and how to make it better for them.  In particular, post-menopausal women should be aware that vaginal dryness can be &#8220;cured.&#8221;"</p>
